Application and Approval Process
Registration is required to use Handi-Transit. For your convenience, a link to the application form is provided below. Detailed instructions are on the first page of the form.
The applicant must complete section A (with assistance if required).
Section B must be completed by a health care professional with full knowledge of your medical condition(s) influencing your mobility.
If you are applying for Handi-Transit services based on a physical disability impairing mobility.
Verification is required by one of these professionals: Chiropractor, Occupational Therapist, Physician, Physiotherapist or Registered Nurse.
If you are applying because you are legally blind.
Verification is required by an Optometrist or Ophthalmologist for individuals who do not have a CNIB Registration Number.
Note: Only these Health Care Professionals can complete the forms required for Handi-Transit.
Health Care Professionals related to the applicant cannot complete the forms.
The information provided by the health care professional is an important, but not exclusive, part of the application process. In addition, Transit assesses the functional ability of each applicant to use the fixed-route transit system.
Application Process
On-Line Application Form
To apply for Handi-Transit services, complete and submit the appropriate version of the on-line application:
1. First time applicants or applicants who have become inactive because they have not used the service recently must complete and submit the full version application form.

2. Current temporary users who are considered short term, and whose mobility status has not improved or who require an extension of service, may complete and submit the short version application.
Here are a few tips to help you successfully complete this process:
- Fill out the application form as directed. Forms that have insufficient information, are unclear or illegible cannot be processed and are returned to the applicant by mail. Handi-Transit is not responsible for items lost in the mail.
- Section A must be filled out by the applicant (with assistance when required).
- Section B must be filled out by a qualified health care professional with full knowledge of the condition affecting your mobility. Only the following Professional Certifications may complete and sign section B of the application:
- Chiropractor
- Occupational Therapist
- Physician
- Physiotherapist
- Registered Nurse
- Don’t forget to sign your application! Applications are most often returned because the applicant’s signature is missing, the Health care Professional portion is incomplete, or the questions on the application form are either unanswered or answered incompletely.
Please note that filling out an application does not guarantee eligibility.
There is no charge to apply for Handi-Transit service. Any fees charged by a qualified health care professional to complete this form are the responsibility of the applicant.
We will review your application and contact you within 10 business days of receipt regarding the status of your request. Additional phone calls or a functional assessment may be required to determine eligibility.
Your file will automatically close if you do not use Handi-Transit service for 18 consecutive months. Once your file has closed, a new application must be submitted to determine your eligiblity for Handi-Transit service.
The personal information collected on application forms is subject to the provisions of the Freedom of Information and Protection of Privacy Act and the Personal Health Information Act. Handi-Transit does not share this information with any person, group or organization.
